Who are the faces of Transitions Healthcare?

Ryan Evans, Administrator, began his career working in a Continuing Care Retirement Community (CCRC) where he served for over 12 years. During his years in the CCRC setting, Ryan participated in multiple aspects of the facility’s operation and management. In 2006 he was afforded the opportunity to obtain his Nursing Home Administrator license, which he obtained that same year. He then went on to operate both their nursing care centers and assisted living units. During this time he developed and implemented a Quality Assessment and Assurance program, which raised the standard of care and improved the quality of care. Ryan went on to operate a long-term care facility in the area taking the facility from a 2-star rating to a 4-star rating. He guided the facility through changes and improvements, which lead them to a deficiency-free nursing survey. He was then approached by the owners of Transitions HealthCare to continue their mission of continued quality care and customer service.


Nancy Allbaugh, RN, CDONA/LTC, Director of Nursing, has been a registered nurse for over 16 years and is a graduate with a dual degree in both business and nursing. Her obvious passion is nursing. Her career started in emergency and intensive care to running an operating room for a Level 1 trauma center. It also includes critical care and home health nursing specializing in hospice and pediatric hospice. Her nitch, however, is skilled and long term care nursing. She has worked as a clinical supervisor and assistant director of nursing for a large, long-term care facility prior to moving to Maryland. She has served on committees to develop policy and procedures for palliative and hospice care in long term care facilities and Quality Assessment and Assurance programs for both medical-related companies as well as standard private businesses. She initiated and opened a ventilator unit for a major long-term care company and has been a preceptor to RNs and LPNs on various nursing skills including ventilators, IVs, TPN, peritoneal dialysis, and wound care. In 2010 she received her certification as a Director of Nursing Administration in Long Term Care.


Dr. Ernesto Mendoza, Medical Director and a member of Family Medical Associates, is currently practicing in Westminster, MD. Dr. Mendoza did his residency in family practice at Prince George Hospital and has been a medical director for 8 years. Dr. Mendoza is also a Member of the American Medical Directors Association.

We make admissions at Transitions Healthcare a worry-free process so that you can concentrate on giving your loved one the support they need. We would be glad to show you around our home. What insurance does Transitions Heathcare accept?

We accept Medicare, Medicaid and most commercial insurances. We are also a preferred provider for CareFirst BlueCross BlueShield.


What does Medicare pay for?

Medicare will pay for up to 100 days of Skilled Care in a nursing facility. The first 20 days are paid at 100%. Days 21-100 includes a co-pay of $144.50 per day. However, if you have additional insurance, this cost may be covered. The average Medicare length of stay in our facility is 32 Days.


How do we apply for Medical Assistance?

Medical Assistance must be applied for in the county where the resident resided before entering the nursing facility. Medical assistance can only be applied for when a person’s assets are under $2500.00 (single) or half the amount of the marital assets subject to a ceiling and a floor ($21,912 effective January 1st, 2009). Please see the Social Service office for assistance.


When are visiting hours?

We are assisting residents with morning care, including showers until 10:00 am daily. Most of our residents start to retire for the day around 8:00 pm. Therefore, we encourage family and friends to visit between the hours of 10:00 am and 8:00 pm. However, there are no set visiting hours for our building.


Do residents have telephone use upon admission?

Yes, residents can make and recieve phone calls. Dial (410) 549-3350 followed by the resident's extension. The extension will consist of the room number you are trying to reach and then the bed number.

  • Bed A = 1
  • Bed B = 2
  • Bed C = 3
  • Bed D = 4

For Example: You are trying to reach room 201, Bed A. The extension will be 2011.


When is the Beauty Shop open?

Our on-site Beautician is available on Tuesdays and Thursdays. Please use the Beauty Shop Request Form (PDF) to schedule an appointment. Please see the front desk for assistance.
